Effective September 1, 2025 (House Bill 4224, 89th Regular Session, Section 181.105, Texas Health & Safety Code)
As a licensed health care provider in Texas, we are required to provide clear instructions to consumers about how to:
- Request your health care records
- Contact the appropriate licensing authority
- File a consumer complaint
1. Request Your Health Care Records
You have the right to request copies of your health care records. To do so:

- Mail a written request to: 1645 Greens Prairie Rd W, Suite 101, College Station, TX 77845
- Submit a request via messaging in your Pain in the Rear View’s Client Portal
Please include:
- Full name
- Date of birth
- Dates of service
- Specific records requested
Your request will be processed in accordance with applicable Texas law and federal privacy regulations (HIPAA).
2. Contact the Licensing Authority
If you need to contact the licensing or disciplinary authority for your provider, please use the following:

3. File a Consumer Complaint
You may file a consumer complaint with the Texas Attorney General’s Consumer Protection Division:

Provide as much detail as possible regarding your complaint, including dates, names, and descriptions of the issue.
